各县（区）人民政府、市直各单位：
为了认真贯彻国家《传染病防治法》，执行国家《生活饮用水卫生标准》。现将《晋城市生活饮用水防护、监督和二次供水卫生管理暂行办法》发给你们，望贯彻执行。
晋城市生活饮用水防护、监督
和二次供水卫生管理暂行办法
根据国家《传染病防治法》的规定和《生活饮用水卫生标准》要求，为了改善和提高我市生活饮用水水质质量，控制肠道传染病的发生和流行，保障广大人民群众身体健康，以适应城乡经济建设高速发展的需要，特制定本办法。
卫生防护
第一条 凡以地下水（包括浅层和深层），地表水（包括河塘、水库）和山泉水作为饮用水源的，必须建立防护距离和防护带，并有明显的标准。以河流作为饮用水源，其取点上游1000米和下游100米水域内不得有任何污染源排入，以其它地表水和地下水作为饮用水源的其水源周围30-50米以内不得有任何污染源。
第二条 凡高层建筑有二次供水设施的单位，水箱构筑物材料要选择合理，必须严格进行防护管理，防止饮月水二次污染。
第三条 医院、皮革生产加工等单位，有严重污染可能的废水排放，必须经过处理，严禁排入渗水并污染地下水源。
卫生管理
第四条 对集中式供水单位，实行“卫生许可证”制度，由县以上卫生行政部门签发、卫生防疫站发放两年复核一次。
第五条 各所属饮用水源的责任单位，确立专人领导和专（兼）职管理人员，要尽职尽责做好工作。
第六条 所有集中式供水和二次供水单位的制水、供水、检验和管理清洗人员必须每年组织进行一次健康检查，体检合格者，由所属市、县（区）卫生防疫站发给健康合格证。凡患有伤寒、痢疾、肝炎、结核和渗出性（含化浓性）皮肤病的一律不得直接从事制水、供水和管水工作。
第七条 无论集中式和分散式供水，必须坚持饮水消毒制度，确立饮水消毒员，并经各所辖卫生防疫站培训合格后方可上岗。
第八条 集中式供水单位，要建立水质档案，制定饮水卫生管理制度，分散式供水要制定饮水管理公约，对二次供水的水箱要定期进行清洗，加盖加锁，必须做到严格管理，使饮水水质达到国家规定的《生活饮用水卫生标准》。末稍水和二次供水水质、大肠菌群、细菌总数、混浊度、余氯四项指标合格率达到100%。
第九条 凡有水质检验条件的集中式供水单位，必须每月向所直接管辖的市、县（区）卫生防疫站报告水质检验资料。
卫生监督
第十条 市、县（区）卫生防疫站，对所直接管辖生活饮用水供水单位实施卫生监督权。
第十一条 各级卫生防疫站对所直接管辖的集中式和二次供水及分散式供水单位的水质要进行定期和不定期的监督监测，掌握水质变化情况，及时发现问题，提供改善水质措施。
第十二条 做好集中式供水单位从业人员的年度健康检查和消毒员的培训工作。
第十三条 对新建、改建、扩建的集中式供水单位和二次供水单位实施预防性卫生监督，在设计、施工、竣工验收时，必须有直接管辖的市、县（区）卫生防疫站参加。
第十四条 本办法由市卫生局负责解释。
第十五条 本办法自批准下发之日起施行。

商业部关于启用新的《市镇居民粮食供应转移证明》及有关使用规定的通知

为了加强粮食供应管理，堵塞漏洞，决定从一九九二年六月
一日起启用新的《市镇居民粮食供应转移证明》（样本附后，以下简称《市转证》），旧的《市转证》同时废止。现就新的《市转证》使用和管理规定通知如下：
一、《市转证》的签发和接收统一由县级或相当于县级粮食部门办理，基层粮店、粮站、粮库不再办理。
二、《市转证》的上端和骑缝处，须盖有县级主管粮食工作的行政单位（含赋予行政管理职能的公司）公章；下端须盖有经办人、复核人名章和签发单位“市转证专用章”方为有效。
三、《市转证》的办理要求：
（一）《市转证》的签发先由经办人填写，加盖个人名章，再由复核人审核无误后加盖个人名章和签发单位“市转证专用章”，实行“双线”办理。
（二）签发《市转证》须用黑色炭素墨水钢笔填写，字迹要工整，不得涂改。迁移人数不足4人，剩下的空格应用笔划掉，同时加盖“以下空白”戳记；迁移人数超过4人，须另外填写新的一页《市转证》。如填写中出现错误，应加盖“作废”戳记，不得撕毁，全部保留备查，重新另起一页填写。
（三）新《市转证》骑缝处有4个顺序号，发证时应沿着迁出人数相应的顺序号下方虚线剪下，迁入方应检查顺序号与迁出人数是否相符。
（四）有关项目的填写方法及说明
1．标题中“省（区、市）”前填写省、自治区、直辖市名称，“市（县）”前空格处填写市（县、区、旗）名称。
2．标题下和骑缝处编号前“××字”的填写，请省、自治区、直辖市粮食局自行排列与确定，并报商业部粮食管理司备案。
3．定量标准——按迁出地实际定量标准填写，迁入地按当地定量标准重新核定。
4．居民身份证号码或出生日期——填写迁移人身份证编号。对尚未领证或未达到领证年龄的人员均填写“未领”，并同时填写出生日期。
5．迁移原因——填写迁移人迁移的理由。如“调动工作”、“投靠亲属”、“升学”等。
6．原住址——填写迁移人迁出前户口所在地的详细地址。
7．迁往地址——填写迁移人迁入地详细地址。
8．有效期——省、自治区、直辖市内迁移的有效期由省、自治区、直辖市粮食局自行规定；跨省（区、市）迁移的有效期，定为由迁出地停止供应之日起，三个月之内有效。
9．本证适用于一般性正常迁移。如迁移临时粮食关系，仍加盖“临时”字戳，按有关规定办理。
四、《市转证》的保管、发放、建档制度按商业部部发（91）粮字第1194号文件规定执行。
五、签发单位的空白《市转证》如遗失、被盗，应及时通知省、自治区、直辖市粮食局，并在《中国商报》上刊登遗失声明宣布作废，重大案件要向公安机关报案。迁移人在迁移过程中发生遗失的，本人应写明情况，职工、干部须凭单位证明，居民须凭街道证明，到原签发单位申请补办。原签发单位查对存根和有关证明，经认真审核无误后，方予补办，并在原存根联加盖“作废”戳记，将有关证明存档备查，将作废号码和补办号码通知迁入地粮食局。
六、新疆、西藏自治区《市转证》同时印有民族文字（样本另发），请迁入地予以注意。
七、新的《市转证》由商业部统一印制。各签发单位在签发《市转证》时，可向迁移人收取合理的工本费。
八、其他事项，仍按现行有关规定办理。
